WSHIP has made applications for plans effective in 2010 available on their website.  Since we are now processing applications for coverage that will be effective in 2010, please begin to use the 2010 version of the WSHIP application immediately.

Also in 2010, WSHIP will be using smoker/nonsmoker rates for the first time.  This change is being made to bring WSHIP's rates in alignment with other health insurance plans available in the market.  Applicants will be asked to state whether or not they have used tobacco products in the last 12 months on their WSHIP application.  Effective Tuesday, December 1, 2009, Evergreen will need this information to process new WSHIP applications.

A Tobacco Use Affidavit Form is available on the WSHIP website.  All current WSHIP clients have been mailed this form as part of their open enrollment paperwork, and it should be returned to WSHIP promptly so that they can properly determine rates for 2010.
